Purpose:
Provides maternal child health nursing services to first-time pregnant women and children in accordance with the Nurse-Family Partnership model.
Responsibilities:

Provides home visiting services to pregnant individuals and children.

Provides strength-based, family-centered, comprehensive care that assists clients in setting goals and reaching an optimal state of health.

Manages caseload in collaboration with the manager.

Provides nursing case management and care coordination to clients and families.

Confers with other agencies regarding client needs; exchanges information and attempts to determine a solution for such needs.

Assists with orientation, instruction, and guidance of new staff members, students, and visitors coming to the agency.

Maintains accurate and timely documentation in clinical and billing records and databases.

Teaches and advises clients and families on a variety of health-related issues such as the prevention of disease, antepartum and postpartum processes and warning signs, infant care, normal childhood growth and development.

Utilizes NFP core curriculum, Partners in Parenting Education (PIPE), Promoting Maternal Mental Health in Pregnancy curriculum, crisis intervention and problem-solving techniques with women, infants, and parenting individuals.

Informs physicians and other personnel of changes in the client’s condition and nursing needs.

Works to maintain a safe environment for the client and/or infant.

Assists the client’s family/caregiver and other health care personnel in providing continuity of care.

Complies with accepted professional standards and ethical guidelines.

Represents EveryStep in a positive and professional manner, communicates effectively and respectfully with team, manager and community partners, maintains confidentiality at all levels of the agency.

Attend regularly scheduled meetings

Manage time, organization, and scheduling to meet client needs as well as other job duties and responsibilities

Utilizes language interpretation/translation services and obtains pertinent information according to the culture and primary language of each individual as appropriate

Engage in on-going personal professional development and training

Other duties as assigned by supervisor
Qualifications:

Bachelor’s degree in Nursing from an accredited school required

Current and valid Iowa RN license

Knowledge of professional nursing principles and practices, including knowledge of family-centered nursing care

Working knowledge of maternal child health nursing principles and practices, including community health problems, preventive measures, and community resources

Ability to plan, organize, and utilize teaching principles in working with families, clients, schools, and other community organizations

Ability to exercise independent nursing judgment

Excellent critical thinking and decision-making skills

Ability work effectively with diverse populations

Ability to communicate effectively, both verbally and in writing, using the English language

Ability to physically carry out the various aspects of providing services in the client’s home, school, or other environments

Ability to successfully complete training as required by the Nurse Family Partnership model

Current American Heart Association Healthcare Provider certification

Mandatory Reporter of Child and Dependent Adult abuse training certification (within 30 days of hire)

Must have valid driver’s license, auto liability insurance and reliable transportation if driving is required

Successful completion of pre-employment physical, drug screen, 2-step TB test and background check
Working Conditions:

Physical requirements: Medium work exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.

Physical activities: Climbing, Kneeling, Reaching, Pushing, Talking, Hearing, Repetitive motions, Balancing, Crouching, Standing, Pulling, Stooping, Crawling, Walking, Lifting, Fingering (picking, pinching, typing, etc.), Grasping (applying pressure with fingers and palm), Feeling (perceiving size, shape, texture, temperature by touching), Visual acuity (field of vision for analyzing data, viewing computer, using measurement devices, operating machines, determining accuracy or neatness, and/or making general observations)

Environment: Extreme temperatures, Hazards (chemicals, infectious diseases)

Time: Ability to be flexible with a varying schedule and programmatic requirements

Travel: Required between locations
